Please make sure you print off both forms to bring in with your payment. All relevant dates are on this pamphlet.

The waiver MUST be signed. If you have questions, please feel free to send a message.

Hours vary at Westside over the next few weeks so please make sure to call 803-775-3484

to verify that someone will be there to receive your registration/payment.

Forms are available at Westside if you do not have access to a printer.
